How it works

A useful distinction

Keep the equation with the result

When comparing eGFR records, record the equation and creatinine units alongside each value. Switching equations can change the estimate even without a biological change; a clinician can interpret a trend using consistent methods.

Selecting an adult or pediatric kidney estimate

The adult and pediatric eGFR modes use different equations and required measurements. Select the appropriate method and verify the laboratory units before entering a result. An estimated filtration rate is not the same as a direct measurement and must be interpreted in clinical context. A single calculated value cannot by itself establish a diagnosis or determine treatment.

Adult CKD-EPI 2021: formula and assumptions

eGFR = 142 × min(Scr/κ,1)^α × max(Scr/κ,1)^−1.200 × 0.9938^age × 1.012 (female).

The 2021 CKD-EPI creatinine equation is race-free. Kidney function interpretation requires clinical context and evidence over time.

Pediatric bedside Schwartz: formula and assumptions

eGFR = 0.413 × height(cm) / creatinine(mg/dL).

The bedside Schwartz estimate is for pediatric use under clinical supervision; it is not the adult CKD-EPI equation.

Worked example

Using the Adult CKD-EPI 2021 example:

Inputs
Sex used by the equation
Male
Age (years)
50
Serum creatinine (mg/dL)
1
Creatinine unit
mg/dL
Method

eGFR = 142 × min(Scr/κ,1)^α × max(Scr/κ,1)^−1.200 × 0.9938^age × 1.012 (female).

What to keep in mind

Results are estimates in mL/min/1.73 m². Do not diagnose kidney disease or change treatment from a calculator result.

Common questions

Does one eGFR result diagnose kidney disease?

No. Interpretation requires clinical context and evidence over time. A calculator result should not be used alone to change treatment.

Can I use the adult equation for a child?

No. The pediatric mode uses the bedside Schwartz relationship with height and creatinine, under clinical supervision rather than the adult equation.
